A nonhormonal melanocortin receptor agonist that works through central nervous system pathways involved in sexual desire.

How Does Bremelanotide Work?

Bremelanotide activates several members of the melanocortin receptor family.

At therapeutic exposure, FDA identifies MC1R and MC4R as the most relevant receptor interactions.

MC4R — Central Nervous System

MC4 receptors are found throughout multiple areas of the brain. Melanocortin signaling within these networks is involved in behavior, motivation, energy regulation, and pathways relevant to sexual desire and arousal.

MC1R — Pigmentation

MC1 receptors are found on melanocytes. Activation can increase melanin expression, which explains why focal hyperpigmentation can occur with bremelanotide.

What We Know—and What We Don’t

The melanocortin system clearly plays an important role in the drug’s pharmacology. However, FDA prescribing information states that the exact mechanism by which Vyleesi improves HSDD remains unknown.

Acquired, Generalized Hypoactive Sexual Desire Disorder

Vyleesi is FDA approved for premenopausal women with acquired, generalized HSDD when low sexual desire causes marked distress or interpersonal difficulty.

The low desire must not be explained by a co-existing medical or psychiatric condition, relationship problem, medication, or another drug substance.

What Does “Acquired, Generalized” Mean?

Acquired means the problem developed after a period in which sexual desire was not problematic.

Generalized means the low desire occurs regardless of the type of stimulation, situation, or sexual partner.

Limitations of the FDA Approval

Vyleesi is not FDA indicated for postmenopausal women, men, or simply to enhance sexual performance.

What Did the Clinical Trials Show?

Two randomized Phase 3 RECONNECT trials established efficacy for the approved population

The pivotal RECONNECT program included two nearly identical randomized, double-blind, placebo-controlled Phase 3 trials.

A total of 1,267 women were randomized, with 1,247 included in the safety population.

Increased Sexual Desire

Both Phase 3 studies demonstrated statistically significant improvements in the Female Sexual Function Index desire score compared with placebo.

Reduced Distress

Bremelanotide significantly reduced the degree to which women were bothered by low sexual desire compared with placebo.

Sustained Research

A 52-week open-label extension found sustained improvement in HSDD symptoms without new safety signals emerging during the extension.

The Clinical Goal

The key outcomes were not simply “more sex.” The trials focused on whether women experienced greater sexual desire and less distress about low desire.

An Important Research Detail

The pivotal FDA analysis did not find a significant difference between Vyleesi and placebo in the change in number of satisfying sexual events. The demonstrated benefit centered on desire and distress.

The Timing Is Individual

FDA notes that the optimal treatment window has not been fully characterized. Patients using the approved product may determine timing based on their own experience with benefit and adverse effects.

Nausea

The most common adverse reaction. In Phase 3 trials it occurred in approximately 40% of Vyleesi-treated patients.

Flushing

Flushing occurred in approximately 20% of treated patients in the controlled clinical trials.

Injection-Site Reactions

Local pain, redness, bruising, itching, or other injection-site reactions occurred in approximately 13%.

Headache

Headache occurred in approximately 11% of treated patients.

Blood Pressure

Bremelanotide can temporarily increase blood pressure and reduce heart rate after administration.

Hyperpigmentation

Focal darkening of areas including the face, gums, and breasts has been reported and may not completely resolve in every patient.

Cardiovascular Contraindication

Vyleesi is contraindicated in patients with uncontrolled hypertension or known cardiovascular disease.

Pregnancy

Use during pregnancy is not recommended. FDA labeling advises effective contraception while taking Vyleesi and discontinuation if pregnancy is suspected.

Oral Medication Absorption

Bremelanotide can slow gastric emptying and may affect absorption of oral medications. FDA specifically advises avoiding oral naltrexone-containing products used for alcohol or opioid addiction because bremelanotide can substantially reduce naltrexone exposure.

Bremelanotide is not a testosterone or estrogen replacement therapy. Its primary pharmacology involves melanocortin receptor signaling.

Is PT-141 the same as Viagra?

No. Sildenafil and similar medications primarily influence peripheral vascular pathways involved in erectile function. Bremelanotide acts through melanocortin signaling within the central nervous system.

Is bremelanotide FDA approved for men?
